Couple: Please enter spouse name, DOB and relationship
Family: Please include spouse and other member names, DOB and relationship

You can remove credit card fees by selecting “Change” next to processing fees and unchecking the box. Also, you can remove Optional Tip to Givebutter and set to $0.

Everyone must complete the form. If you’re paying by Cash/Check/ Zelle, please select “Check” in the payment method.
